• What is your fitness background?
• What is your current education level?
• What country and state do you currently reside in?
• How old are you?
• What apparel out of our collection are you interested in repping?
• If accepted into the program do you agree to allow us to use your content for marketing purposes?
• Do you have any additional questions?
